As our customer, you have already provided us with some private information. We may also collect private information, either internally or through third parties, which we deem necessary to manage our business or service your account. Protection of your privacy is important to us, and we are committed to treating your private information responsibly. For example, we maintain physical, electronic, and organizational safeguards to protect your private information, and we constantly monitor their effectiveness.
We will advise you of our policies for collecting, using, protecting, and disclosing your private information when you first become our customer and every year thereafter that you remain our customer.

Here are some examples of the sources and types of private information we may collect:
- From you: information from applications or other forms, such as names, addresses, telephone numbers,
drivers license numbers, social security numbers, and other information relevant to underwriting your
business, administering your business, or processing a claim;
Although we do not sell your private information, we may disclose any of that collected information:
- To affiliated or nonaffiliated third parties, such as your insurance company, a fraud prevention bureau,
or medical information bureau; and
- As required or permitted bylaw.
We do not, however, disclose your private information to any nonaffiliated third party for its own marketing purposes. Any disclosure to a nonaffiliated third party is incidental to administering your certificate or claim with us and permitted by law. Furthermore, we treat private information of former and prospective customers in the same manner as existing customers.
- From transactions with us:
payment record, underwriting transactions, and claims; and
- From third parties: prior claim data, medical records and reports, and other information relevant to underwriting your business, administering your business, or processing a claim.
